We also need your assistance with information to ensure that I have all of the details relevant to your situation, so we can email you our New Patient Registration Form and Spinal Situation Questionnaire.
Please arrive at least 15 minutes before your first appointment so that we can collect any needed information from you. When you come for your appointment please remember to obtain and bring the following:
- Referral letter from GP;
- All x-rays, scans and other investigations;
- A list of all your medications including any over-the-counter medications or herbal supplements;
- Medicare and other relevant cards (DVA);
- Your health fund details;
- WorkCover patients require your insurer and claim number information.
